Our Insurance carriers no longer accept the “Student Verification of Full Time Status” forms which were used in the past. Each carrier has its own requirements for student verification. Below is the procedure for verification of full-time student status with each:
United Health Care : 877-769-7447 Option # 1
You must write a brief letter to United Healthcare stating your name, ID number, your child’s name and ID number, the name of the college which your child attends and the address of the college. Fax the letter to: 845-336-7989

Blue Cross/Blue Shield : 877-769-7447 Option # 2
The required form to be sent to Blue Cross Blue shield can either be requested via phone, or, you may download from their website at www.empireblue.com ; click on “College-Student Dependent Health Benefits” link and follow the directions. Fax the completed form to: 1-800-780-1224
